I declare that I will adhere to the SRLPDC’s Principles of Professional Practice Policy and Continuing Professional Development Policy.

I declare my commitment to maintaining and developing my competence and expertise through continuing training and agree to engage in a programme of Continuing Professional Development of which I am keeping an up to date record. I am aware my CPD must be submitted in full by 30.9.24.

I am a member of the PVG Scheme.

I have current Professional Indemnity Insurance cover (please attach copy of your policy for evidence)

PERMISSION/CONSENT FORM

I give consent for SRLPDC to upload my name and registration number to the SRLPDC online directory (for purposes of verification of SRLPDC registration to external parties).

I give my consent for copies of my qualifications and photo to be retained by the SRLPDC Office to support ID queries.
